To help ensure the health and safety of our young athletes, the Centers for Disease Control developed the Heads Up: Concussion in Youth Sports initiative to offer information about concussions to coaches, parents, and athletes involved in youth sports.

BACKGROUND CHECK DISCLOSURE

Rosemount Area Athletic Association (the “Organization”) may order a “consumer report” (a background report) on you in connection with your volunteer application, and if you are accepted as a volunteer, or if you already volunteer for the Organization, may order additional background reports on you for volunteer purposes, to the maximum extent permitted by applicable law.

The background check company, ADP Screening and Selection Services, will prepare the background report for the Organization.  ADP Screening and Selection Services is located at 301 Remington Street, Fort Collins, CO, 80524, and can be reached by phone at 800-367-5933 or at their Internet Web site address www.adpselect.com.

The background report may contain information concerning your criminal history.  Information may be obtained from private and public sources.
